Managed Care - Insurance Filing
Once you have an appointment, you may need to obtain authorization from your primary care physician (depending on your insurance plan). Our scheduling staff will know what authorization, if any, needs to be obtained. Please understand that it is the patient's responsibility to obtain authorizations. Please bring your insurance card to your appointment. We will file all insurance claims.
Please come to your appointment prepared to pay your co-pay, your deductibles and other patient responsibility amounts. These payments are due at the time services are rendered. We accept cash, checks, Visa, Master Card, Discover and American Express.

Prescriptions and Refills
Remember the doctor does not have access to the medical record at night or on weekends so prescription requests must be handled during routine business hours.
We will need basic information to process your request:
- Your child's name with correct spelling
- Date of birth
- Weight
- Any allergies your child might have
- The phone number of your pharmacy
- State clearly the drug you are requesting and the amount you give and how often you give the drug
Prescriptions are routinely called in by the end of the day, but please allow two days for this process to be completed, in case there are difficulties. Be aware that preparing some liquid or compounded medicines may take an additional day or two.
